چکیده

the basis of the following characteristics: catalase negativity, α-hemolytic gram-positive cocci forming pairs and tetrads (not chains) in broth culture; growth in the presence of 40% bile and 6.5% NaCl and ability to hydrolyze esculin; pyrrolidony l-aminopeptidase positivity, leucineaminopeptidase negativity; and production of acid from trehalose, sucrose, maltose, and lactose but not from sorbitol. Susceptibility testing by the agar dilution method showed that the isolate was susceptible to penicillin-G (MIC = 0.12 μg/ml) and vancomycin (MIC = 0.25 μg/ml). On the basis of this case and previous reports (1,2), we believe that A. viridans is a potential pathogen that can cause serious infections in immunocompromised patients. The presumed route of infection in this patient was esophageal ulcers. Clinical microbiologists should pay close attention to α-hemolytic, catalasenegative streptococci recovered from sterile body sites that form tetrads rather than chains on Gram stain.

Proficiency of clinical laboratories in and near Monterrey, Mexico, to detect vancomycin-resistant enterococci.

Early detection of vancomycin-resistant enterococci is important for preventing its spread among hospitalized patients. We surveyed the ability of eight hospital laboratories in and near Monterrey, Mexico, to detect vancomycin resistance in Enterococcus spp. and found that although laboratories can reliably detect high-level vancomycin resistance, many have difficulty detecting low-level resist...

Proficiency of determination of vancomycin susceptibility in enterococci by clinical laboratories in Taiwan.

Eighty clinical microbiology laboratories in Taiwan were evaluated for proficiency in the determination of vancomycin susceptibility of enterococci. Each laboratory was given 1 vancomycin-sensitive isolate and 3 vancomycin-resistant enterococci (VRE) isolates to determine the levels of vancomycin resistance. Among a total of 240 tests performed, 153 (63.8%) correctly determined the levels of va...

Proficiency of clinical laboratories in Spain in detecting vancomycin-resistant Enterococcus spp. The Spanish VRE Study Group.

Studies in a variety of U.S. clinical laboratories have demonstrated difficulty in detecting intermediate and low-level vancomycin-resistant enterococci (VRE). The misclassification of "at least intermediate resistant isolates" as vancomycin susceptible may have both clinical implications and a negative impact on measures to control the spread of VRE. No published study has assessed the ability...

Are clinical laboratories in California accurately reporting vancomycin-resistant enterococci?

In order to determine whether hospital-based clinical laboratories conducting active surveillance for vancomycin-resistant enterococci in three San Francisco Bay area counties (San Francisco, Alameda, and Contra Costa counties) were accurately reporting vancomycin resistance, five vancomycin-resistant enterococcal strains and one vancomycin-susceptible beta-lactamase-producing enterococcus were...

Antimicrobial susceptibility testing: special needs for fastidious organisms and difficult-to-detect resistance mechanisms.

Clinical microbiology laboratories are faced with the challenge of accurately detecting emerging antibiotic resistance among a number of bacterial pathogens. In recent years, vancomycin resistance among enterococci has become prevalent, as has penicillin resistance and multidrug resistance in pneumococci. More recently, strains of methicillin-resistant Staphylococcus aureus with reduced suscept...
